DEED POLL - Deed Poll to abandon the use of Hanyu Pinyin name and to add a Christian name

I have a Hanyu Pinyin name on my identity card and I want to abandon the use of my Hanyu Pinyin name.  At the same time, I have a baptism certificate and I want to add my Christian name onto my identity card.
